因新冠疫情持续恶化，继大阪府、兵库县、宫城县3地之后，4月12日东京都的部分地区、京都府京都市和冲绳县那霸市也正式开始实施“蔓延防止重点措施”。该措施被称为“准紧急事态宣言”，餐厅等商家在晚间8点之后将不得进行堂食营业，拒不遵守的将被处以20万日元以下的罚款。
东京都的“蔓延防止重点措施”实施期为4月12日至5月11日，实施范围包括东京都23个中心城区，以及西部郊区的八王子市、立川市、武蔵野市、府中市、调布市、町田市6个市。京都市和那霸市的“蔓延防止重点措施”将于5月5日结束。
【日本首现接种2次疫苗后仍感染病例】
日本石川县立中央医院一名50多岁的女性医务人员，在完成全部2次新冠疫苗接种后，仍然感染了新冠病毒，这是日本首次发现接种2次疫苗后感染的病例。在此之前，日本另一名20多岁的女性医务人员在2月下旬接种第一剂疫苗6天后确诊感染。
石川县这名女性分别于3月13日和4月3日接种了美国辉瑞公司的新冠疫苗，不久后其家人确诊感染，其本人在进行筛查后也于4月10日确诊感染，但是没有症状。
日本厚生劳动省称，接种疫苗会降低感染风险，但并不意味着可以绝对阻断感染。石川县政府则称，这名女性感染后没有出现症状，可以认为是疫苗起到了降低重症化风险的作用，不过也有可能她在接种疫苗前就已经感染。
